Log Of Sexual Harassment

Just so I remember it all:

-Called me into his office to berate me for "not making enough eye contact"

(Date Unknown)
-Berated in office for "being quiet and a private, reserved person". Implied I would not get a raise if my "performance didn't improve". I show up early, leave late, sometimes don't take a break in an 8-10 hour day because I'm working so hard...so I'm pretty sure my performance was not the issue.

-Came in and said "I forgot to bring a shirt (for under the jacket)....so I guess you'll have to just look at my chest hair."

-Told me he'd give me a raise "so I could buy shirts that come all the way down to my pants". I wear scrubs at work so what I wear is not his concern. He continued to comment about it for the rest of the day and days following.
-Asked me about modeling and "what is the weirdest comment anyone has ever made to you?" Probably one of yours.

-Did not go into work because I had a bad weekend and was not in the mood to be creeped out.

9/12/07
-Supposed to be alone in the office, cleaning and answering phones. He came in to "do work" which can all be done during normal office hours.
-Asked about my sleep habits. I told him I don't sleep well. He asked if anyone I sleep with has ever told me I stop breathing in my sleep. (He always says "I'm not trying to offend you" or "I'm not trying to get into your personal life" beforehand)
-Asked about modeling. Asked if I ever have done or would do nude modeling. I said no and I wouldn't.
-Told me the most notable thing about me is my boobs.
-Told me he used to do photography and took naked pictures...went into GREAT detail about the kinds of pictures he took. Told me he "still has his photography equipment".
-I told him I wouldn't do anything nude or pornographic, ever. He started telling me about a porn he once saw with a famous actress in it, and "it didn't hurt HER career."
-Said he overheard me telling one of the other girls that I couldn't have children. Asked why. I didn't want to get into it (I didn't want to talk at all...I wanted to go back to work and work in peace and quiet). He asked if it was a problem with my uterus or my ovaries. I said I didn't know. He kept saying how sad it is that I can't have kids and kept asking why.
-Asked about my health. Called me "a stick with boobs".
-Asked about my religion. Told him I am Seventh Day Adventist. Once AGAIN brought up the shirt that didn't meet my pants (the shirt was less than a centimeter above my pants). Somehow it was brought up about how I tried to fast for Lent. I told him I had lost some weight during then, at which point he leaned forward and grabbed my leg.
-Started telling me about how he doesn't get along with his wife, etc.
-Asked for my phone number (this was fine since we need the other girl's numbers in case someone calls in sick, etc, so I gave it to him). He said "I promise I won't call you late at night and ask you what you're wearing."

-Told me to go on break, but I was in the middle of sterilizing instruments and did not want to go. I feel uncomfortable in the break room because I can't look at my phone without him asking me if I'm "talking to my boyfriend" (although I don't remember the date that occured) and I want to have a break where I can sit and relax without wondering what weird comment will be made next....so I don't like to go on break. I told him I didn't want to go and I would only throw up due to stress if I ate anyway. He demanded that I bring in a doctor's note telling him "exactly what I can and can't due during work", which I feel is none of his business. My medical issues do not affect my work performance and he has no right to pry that deeply into my personal life unless it was a conflict, which it most certainly is not.
